I recently opened the card to take a picture of the heatsink. The OC was usually working 100% with no crashes. Now it crashes very 3 min. What is the problem? The temps looks fine.

Lower clocks, or raise voltages every crash. If this doesn’t eventually keep it from happening replace riser.
I completely removed my OC - now the fan is just set to 75%, and it is still happening occasionally (every few hours). I will try to increase the voltages.
No oc is just as bad as bad ocs most cases.
